PICO is a framework for turning a clinical question into a structured, answerable format. It stands for Population, Intervention, Comparison, and Outcome. Defining your question precisely is not just a formality — in GRADE, certainty applies to a specific answer for a specific outcome, not to a paper in isolation. The same study can yield different certainty ratings depending on how closely it matches your question.

What is the main intervention, treatment, or exposure?
What is it compared to? (Placebo, standard care, another treatment)
